Our office has gone PAPERLESS.  We require all patients to complete the Check In Packet every year.  Two days prior to your appointment, we will send a link to a compatible cell phone or email address on file.  Click on the link to complete the check in forms and medical history questionnaire.   If you have trouble or do not have a way for us to send the forms to you electronically, our office staff will be happy to go through the process with you in the office - we just ask that you plan to arrive 15 minutes before your scheduled appointment to do this.
